 Foot Care of Northeast Arkansas is a provider for Medicare and most major insurance plans. We encourage all patients to confirm their coverage prior to making an appointment.
The Foot Care of Northeast Arkansas charges for services are what we determine to be normal and customary fees for the services we provide. It would be our pleasure to provide any necessary information for your insurance claims related to any treatments or surgeries that were preformed in our office. However, being that there is such a wide variation amongst insurance companies concerning benefit fees and the sheer number of insurance companies out there, we have no way of knowing what is covered by your insurance company. Although we will verify your coverage, it is YOUR responsibility to contact your carrier since the agreement is between you and your carrier to determine coverage for our professional services. In additions, any charges accrued that are not covered by your insurance company will be your responsibility. Also, any claims not paid by your insurance company within 45 days of receiving services will become your responsibility.
ALL INSURANCE INFORMATION YOU PRESENT MUST BE ACCURATE AND CURRENT. PROVIDING THIS INFORMATION IS YOUR RESPONSIBILITY. YOU WILL BE BILLED FOR OUR PROFESSIONAL SERVICES IF THE INFORMATION IS NOT CORRECT. IF YOU CHANGE INSUARNCE COMPANIES, YOU MUST NOTIFY FOOT CARE OF NORTHEAST ARKANSAS.
The federal government can prosecute us if we do not charge and collect the co-payment, therefore your co-payment will be collected before services are rendered for HMO's and PPO's. Deductibles, if applicable, must also be paid at this time.
Insurance companies may also require pre-certification, pre-determination, or a second opinion. For your convenience, X-rays and any necessary records will be provided, however, we require prior notice of at least 3 to 5 days for duplication.
No original x-rays are released to patients. No Exceptions.
